You will be always treated with respect and care.
Disclosure of all information during therapy and consultations remains confidential with the exceptions discussed in other documentation regarding safeguarding, legal requests and sharing with other professionals with your consent.
If receiving medical treatment of any kind, it is recommended that proper diagnosis is sought where relevant, to assist the therapist and to inform those professionals of your enquiries toward Hypnotherapy.
Treatment Consent
MHS has fully explained the procedures and therapy.
Hypnotherapy relies on the commitment and motivation from both of us to get the best results. This includes your full commitment in the process, regular attendance, application of learnings from the sessions into your everyday life and listening daily to a hypnotherapy recording.
I accept the fee payable and note the 24-hour notice of cancellation of any appointment that is required, otherwise the full fee will be charged.
MHS reserves the right to refuse or postpone therapy if feeling threatened, disrespected or feels that therapy is not appropriate for any reason.
I accept that results vary from individual to individual and as for any therapy, there is no 100% guarantee of success.
